Patient Care Facilitator II

Patient Care Facilitators play a crucial role in our organization's mission to improve patient outcomes. As a Patient Care Facilitator II, you will work closely with our interdisciplinary care teams to ensure high-quality, cost-effective care delivery.
Responsibilities- Coordinate care plans for individual patients, considering medical, behavioral, social, and community-based services.
- Collaborate with healthcare providers, caregivers, and other stakeholders to address patient needs and disease processes.
- Develop and implement care plans, monitoring patient progress and making adjustments as needed.
- Bachelor's degree in a relevant field or Associate's degree with 3 years of additional experience.
- 2 years of experience in healthcare or a related field.
- Valid driver's license and clean driving record required.
