HouseCalls NP/PA

We are seeking a skilled NP/PA to join our HouseCalls team in Dallas County, IA. As a member of our team, you will conduct in-home assessments, perform evidence-based practice screenings, and identify diagnoses to inform care management.
Key Responsibilities- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform point-of-care testing and evidence-based practice screenings as appropriate.
- Identify diagnoses and formulate a list of current and past medical conditions using clinical knowledge and judgment.
- Communicate findings to inform member's primary care physician of potential gaps in care.
- Educate members on disease processes, medication, and compliance.
- Active NP license or ability to obtain by start date.
- APCs must have obtained approval to practice autonomously or without formal supervision from their licensing board, if applicable.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and various dwelling conditions.
- 1+ years of clinical experience in their highest level of education, clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, beyond your assigned region, up to 5% of the time.
