Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team as a float position. The successful candidate will provide high-quality patient care and support to our special needs patient population and their caregivers across all of our FQHC locations.

Key Responsibilities
  • Prepare daily assignments for nursing staff
  • Collaborate with the Director of Nursing to establish and implement systems for day-to-day medical and nursing operations
  • Participate as a member of the Patient Centered Medical Home Care team, including daily huddles and practice team meetings
  • Ensure staff compliance with all aspects of the PCMH initiatives
Requirements
  • Licensed and registered in New York State
  • Current BLS Certification
