Job Summary:

We are seeking a Part Time Community Health RN to join our team at Providence Medford Medical Center. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care and promoting health and wellness in our community.

Responsibilities:

  • Provide patient-centered care to individuals and communities in need.
  • Develop and implement health education programs to promote healthy behaviors and disease prevention.
  • Collaborate with healthcare teams to coordinate care and services.
  • Conduct health assessments and develop care plans to meet the unique needs of patients.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.

Requirements:

  • Graduation from an accredited nursing program.
  • ~ 1 year of Nursing experience.
  • Current RN licensure in the state of Oregon.
