Compensation and Benefits

The successful candidate can expect a market-competitive salary range of $55,000 to $65,000 per annum, depending on qualifications and experience. Job Summary

As a Community Health Navigator with Mercy Flights Inc, you will be responsible for providing exceptional patient care and support services to individuals and families in need of healthcare services. This will involve working closely with multidisciplinary teams, including healthcare providers, social workers, and case managers, to ensure seamless care coordination and access to essential resources.

Key Responsibilities
  • Provide high-quality patient care and support services, adhering to clinical operating guidelines and policies.
  • Maintain comprehensive electronic client files, including client notes, releases of information, assessments, and other medical documents.
  • Complete patient care reports in a timely and accurate manner, in compliance with Oregon State law and company protocols.
  • Participate in ongoing training and education to enhance clinical skills and knowledge.
  • Build and maintain strong relationships with clients, healthcare providers, and community partners, demonstrating exceptional communication and interpersonal skills.
Requirements
  • A High School Diploma or equivalent is required; an associate degree in healthcare or human services is preferred.
  • Oregon Driver's License is a must.
  • CPR certification is essential.
  • At least one year of healthcare or social services experience is required; two years is preferred.
  • Experience in the substance abuse and/or mental health field is a plus.
  • A Community Health Worker certificate is required within one year of start date.
