About the Role

We are seeking a skilled Care Coordinator to support our clients with resource navigation, benefits coordination, and case management. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with our program manager and other team members.

Responsibilities:
  • Manage a caseload of clients and provide individualized support and guidance.
  • Collaborate with the program manager and other team members to develop and implement client care plans.
  • Facilitate group therapy sessions and provide supportive counseling services.
  • Coordinate benefits and services for clients, including healthcare, housing, and employment support.

Requirements:
  • Bachelor's Degree in Psychology, Social Work, Counseling or a related field OR one-year experience in the behavioral health field for non-related Bachelor's degrees.
  • At least two years of experience working with adults in an outpatient setting, preferably in a mental health or substance abuse context.
  • Familiarity with clinical modalities: Motivational Interviewing (MI) and Dialectical Behavior Therapy (DBT), other Cognitive Behavior Therapies (CBT).
  • Strong organizational and time management skills, with ability to prioritize multiple tasks and deadlines.
