**Job Summary:** Nurse Care Coordinator for IDD Clients

FRIENDS of Broomfield is committed to providing high-quality care to adults with intellectual and developmental disabilities. We seek a skilled Registered Nurse to serve as a care coordinator, ensuring our clients receive personalized attention and support.

The ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for delivering person-centered care. This is a full-time position that requires a Colorado Nursing License and three years' experience working with adults with IDD. The estimated salary for this role is $80,000 - $95,000 per year.

Responsibilities:
  • Develop and implement individualized care plans for clients with IDD
  • Collaborate with multidisciplinary teams to ensure effective care delivery
  • Monitor client health status and provide timely interventions as needed
  • Communicate effectively with families, guardians, and other stakeholders regarding client care
