Medical Care Coordinator for Adults with IDD

FRIENDS is seeking a dedicated Registered Nurse to oversee the medical care of adults with intellectual and developmental disabilities. As a Medical Care Coordinator, you will play a vital role in ensuring the health and well-being of our clients. Your primary responsibility will be to coordinate and implement individualized healthcare plans that address the specific medical needs of each client.

The ideal candidate will have at least three years of experience working with adults with IDD and possess excellent communication and interpersonal skills. You will work closely with our multidisciplinary team to provide person-centered care and ensure seamless coordination with medical professionals, hospitals, and other external entities.

Key Responsibilities:

  • Coordinate and implement individualized healthcare plans for adults with IDD
  • Collaborate with medical professionals, hospitals, and other external entities to ensure comprehensive care
  • Provide training and education to staff on medical protocols, health care plans, and other related topics
  • Advocate for the best interest of clients by working with providers to ensure timely and effective care
  • Maintain accurate and detailed documentation of client care and services provided
  • Perform other duties as assigned or requested

Requirements:

  • Curent Colorado Nursing License in good/active status
  • Three years' experience working with adults with IDD
  • Excellent written and verbal communication skills
  • Able to maintain detailed documentation and records
  • Valid Colorado driving license, good driving record, and reliable transportation

Working Conditions:

This position requires a strong commitment to delivering high-quality care and services to individuals with IDD. You must be able to work independently in the community and travel to the FRIENDS office as needed.

We offer a competitive salary range of $80,000 - $110,000 per year, commensurate with experience.
