We are seeking a highly skilled and compassionate Patient Care Manager to join our team at Ochsner Home Health of New Orleans. As a Patient Care Manager, you will play a vital role in ensuring the delivery of high-quality patient care and services to our patients in their homes.
About the RoleThe Patient Care Manager is responsible for the supervision and coordination of clinical services, providing and directing provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
Key Responsibilities- Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and plots start of care visits.
- Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate.
To be successful in this role, you must have:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- At least one year of clinical experience as a Registered Nurse.
