Job Summary:

The Post-Hospital Transition Navigator will play a pivotal role in ensuring the seamless transition of patients to post-acute environments. This position will collaborate with providers, care managers, and social work teams on transitions of care.

This position will be responsible for discharge phone calls, communication of test results, facilitation of patient-provider communication, and coordination of follow-up appointments when appropriate.

This position will report directly to the Director of Care Management.

Key Responsibilities:
• Collaborate with healthcare providers and care teams to ensure smooth patient transitions
• Coordinate discharge phone calls and communicate test results to patients and families
• Facilitate patient-provider communication and coordinate follow-up appointments as needed
• Work closely with the Director of Care Management to ensure effective care transitions

Requirements:
• BSN required; Associate degree with significant experience considered
• Three years or greater of medical-surgical inpatient nursing care experience required
• Three years or greater of inpatient or outpatient care coordination/case management/discharge planning experience required
• Bilingual or Trilingual preferred; ability to utilize interpreter services resources for patients needing services in other languages
• Strong working knowledge of Microsoft Office products required
• Working knowledge of database management and reporting
• Solid problem-solving skills and attention to detail
• Ability to manage complex information and prioritize tasks effectively
