Join Our Team as a LPN Liaison

ChristianaCare HomeHealth is seeking a dedicated and compassionate LPN Liaison to join our team in Southern Delaware. As a key member of our care coordination team, you will play a vital role in ensuring a smooth transition from acute care to post-acute settings for our patients.

Key Responsibilities:
  • Assist referral sources in obtaining demographic, insurance, and clinical information for patients.
  • Meet with patients and families to assess home care needs and develop personalized care plans.
  • Acquire, enter, and update patient consent, advance directive information, and insurance authorization.
  • Communicate with physicians, discharge planners, and case managers to ensure seamless patient care transitions.
  • Coordinate with departments to schedule start-of-care services and meet patient needs.
Requirements:
  • LPN licensure in the State of Delaware or an equivalent state.
  • 1 year of experience in home health, hospice, or care coordination.
  • Knowledge of ChristianaCare HomeHealth service lines.
