* Build and maintain relationships with local healthcare providers, senior services, and community organizations to ensure seamless care coordination.
* Provide education, training, and resources to families, treatment teams, and ACCS staff to promote successful aging in place.
Requirements
* Bachelor's Degree required.
* At least three years of experience working in elder care, either in the community or in a facility.
* Valid driver's license and reliable vehicle required.
