Care Coordination Specialist Opportunity
Optum Home & Community Care is seeking a skilled Care Coordination Specialist to join our team. As a Care Coordination Specialist, you will play a critical role in ensuring the seamless delivery of care to our patients. This is an exciting opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Primary Care Delivery: Deliver high-quality, cost-effective care to assigned patients, managing both medical and behavioral conditions effectively.
- Care Coordination: Coordinate care as patients transition through different levels of care and care settings, ensuring that their needs are met and their wishes are respected.
- Program Enhancement: Regularly communicate with internal and external parties, including physicians, patients, and other provider groups, to promote the CCM program and maintain membership growth.
Requirements:
- Certified Nurse Practitioner or Physician Assistant through a national board.
- Active and unrestricted license in the State of Connecticut.
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- Ability to lift a 30-pound bag in and out of a car and navigate stairs and a variety of dwelling conditions and configurations.
- Availability to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours.
Preferred Qualifications:
- 1+ years of hands-on post-grad experience within Long Term Care.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
- Proficient computer skills, including the ability to document medical information with written and electronic medical records.
