Clinical Manager Job Description

At CenterWell, we're committed to putting health first for our patients, teammates, and communities. As a Clinical Manager, you'll play a vital role in coordinating and overseeing direct care patient services provided by clinical personnel.

Key Responsibilities:
  • Develop and implement clinical operations plans for a specific location, ensuring efficient and effective care delivery.
  • Conduct assessments and reassessments of patients, updating care plans and interpreting patient needs in accordance with Company and physician procedures.
  • Manage the assignment of caregivers and oversee the delivery of care to all patients served by the location.
  • Collaborate with clinicians to promote effective performance and delivery of quality home care services.
  • Assist clinicians in establishing therapeutic goals, setting priorities, and developing patient Plans of Care (POCs).
  • Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers.
  • Coordinate communication between team members, attending physicians, and caregivers to ensure appropriateness of care and outcome planning.
  • Carry out supervisory responsibilities in accordance with Company policies and procedures.
  • Participate in the interviewing, hiring, training, and development of direct care clinicians.
  • Assess staff education needs and create regular staff education programs as needed.
  • Work with Utilization Review staff to analyze data and determine efficiency and efficacy of case management systems.
Requirements:
  • Graduate of an accredited School of Nursing.
  • Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
