RN Case Manager

Good Shepherd Hospice is seeking a compassionate and skilled Registered Nurse Case Manager to join our team. As a RN Case Manager, you will be responsible for providing high-quality patient care and support to patients and their families in Sugarland, Richmond, Rosenberg, Katy, and surrounding communities.
Key Responsibilities:- Provide nursing assessment, planning, and care to maximize the comfort and health of patients and families.
- Respond to all phone calls promptly and appropriately, making patient visits as necessary and completing after-hours admissions and deaths as needed.
- Initiate communication with attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Maintain regular communication with the attending physician concerning patient/family care.
- Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the Plan of Care.
- Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
- Instruct the patient and caregiver on medication administration, personal care, oral hygiene, lifting, moving, ambulation, exercise, disaster/safety management, and preparing for events preceding death.
- Inform the Patient Care Manager of unusual or potentially problematic patient/family issues.
- Attend Hospice team meetings and other patient conferences, including care plan meetings, as deemed necessary by the Patient Care Manager.
- Provide appropriate support at time of death.
- Evaluate hospice aides every two weeks and LPNs at a minimum of one time per month.
- Develop understanding of hospice philosophy through individualized studies.
- Become proficient in pain management through individualized reading, in-services, or seminars.
- Registered Nurse currently licensed in the state in which the care is provided.
- Hospice experience preferred.
- Must be of high integrity, including maintenance of confidential information, including all patient records.
- Works as a team player, following directions, and adhering to defined care plans.
- Demonstrated commitment to the hospice philosophy of care.
- Strong organizational and self-management skills.
- Critical thinker with good judgment.
- Valid Driver's License.
