About the Role

BAYADA Home Health Care is seeking a skilled Registered Nurse to join our team as a Home Health clinician in South Strafford, Vermont. This is an excellent opportunity for a motivated and compassionate nurse to make a meaningful difference in the lives of clients.

Job Summary

This position involves providing high-quality patient care in a home setting, with a focus on delivering personalized support and improving health outcomes. As a clinical nurse specialist, you will be responsible for assessing client needs, developing and implementing care plans, and collaborating with healthcare teams to ensure seamless transitions of care.

Key Responsibilities
  • Conduct comprehensive assessments of clients' physical, emotional, and social needs
  • Develop and implement individualized care plans that address client goals and priorities
  • Provide direct patient care, including medication administration, wound management, and other treatments as needed
  • Collaborate with healthcare teams to coordinate care and ensure effective communication
  • Maintain accurate records and documentation of client care and progress
Requirements

To be successful in this role, you will need:

  • A current license as a Registered Nurse in Vermont
  • A minimum of one year of recent, verifiable clinical experience
  • Excellent assessment and critical thinking skills
  • Strong communication and interpersonal skills
  • Ability to work independently and collaboratively as part of a team
