Our team is comprised of extraordinary professionals who are passionate about delivering exceptional patient care. As a Health Coach, you will play a vital role in our healthcare team, assisting in patient care management and care transitions, screening and connecting patients with community resources, and providing individual support and assistance in navigating the healthcare and social service systems.
Key Responsibilities
- Assist patients in managing their care and transitioning between healthcare settings
- Screen and connect patients with community resources, such as food and housing assistance
- Provide individual support and assistance in navigating the healthcare and social service systems
- Document patient interactions in electronic health records
- Escalate clinical questions to office nurses or Director of Clinical Practice
- Collaborate with clinicians, office staff, population health team, and community partners to ensure continuity of care
Requirements
- Experience working in a healthcare office setting or community outreach and education
- Knowledge of health promotion, nutrition, and counseling (specific disease and lifestyle-related topics ideal)
- Strong interpersonal skills and ability to work well with people of various backgrounds, ethnicities, and life experiences
- Ability to take direction, follow through with tasks, and work independently
- Flexibility, optimism, and a cooperative working style
Education Requirements
- High School Diploma or Equivalent; clinical training (MA, CNA, LPN) a plus
- Associate degree preferred
Special Requirements
- Fluency in Spanish preferred
- Competent in basic computer skills; knowledge of using an electronic health records system preferred
