The Community Support Specialist (CSS) operates under general supervision, exercising moderate discretion and independent judgment.

The CSS reports to the Regional Program Supervisor and works collaboratively with both internal and external stakeholders to enhance awareness of CPP initiatives and provide expertise in various HHSC benefits programs.

Key responsibilities include:

  • Partner recruitment and retention
  • Program support and training
  • Data collection, assessment, and program development
  • Other related duties as assigned

Community engagement may require flexibility in working hours, and travel within the assigned region is expected. The CSS will also participate in various duties necessary for maintaining efficient operations.

Essential Job Functions:

Regular attendance and adherence to agency leave policies are essential, along with the willingness to utilize a personal vehicle for travel approximately 65% of the time.

Partnership Recruitment and Retention (35%):

Conduct assessments of governmental, faith-based, and community organizations within the assigned region. Establish yearly recruitment objectives based on these assessments and work collaboratively with the Regional Manager. Engage and educate local organizations to recruit new Community Partners while maintaining comprehensive records of recruitment activities.

In alignment with the recruitment strategy, the CSS will deliver presentations, identify potential community partners, and meet annual recruitment targets.

Program Support and Training (35%):

Provide ongoing support, training, and technical assistance to community partners. This includes troubleshooting, in-person visits, remote support, and the facilitation of stakeholder meetings to deliver program updates and training.

Data Collection, Assessment/Evaluation, and Program Development (20%):

Document all program activities in the designated database. Participate in ongoing evaluations and quality improvement initiatives to enhance CPP operations.

Other Duties (10%):

Perform additional tasks as assigned to ensure the smooth functioning of the department.

Knowledge, Skills, and Abilities:
  • Understanding of HIPAA and related privacy regulations
  • Familiarity with HHSC Community Partner Program
  • Knowledge of SNAP, TANF, Medicaid, CHIP, and related programs
  • Skills in effective communication and coordination with diverse groups
  • Proficiency in Microsoft Office applications and ability to navigate various databases and systems
  • Ability to work independently and maintain professional relationships with stakeholders.
