Care Management Specialist II

Join L.A. Care Health Plan as a Care Management Specialist II and play a critical role in ensuring our members receive the best possible care. As a member of our care management team, you will work closely with members, families, and healthcare providers to develop and implement personalized care plans that address the unique needs of each individual.
Key Responsibilities:
- Conduct comprehensive assessments of members' health and social needs
- Develop and implement care plans that address members' complex health and social needs
- Collaborate with healthcare providers and community resources to ensure members receive timely and effective care
- Provide education and support to members and their families to promote health and wellness
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
- Associate's degree in nursing or social work
- Minimum of 3 years of experience in care management or a related field
- Current knowledge of clinical standards of care and disease processes
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
