Job Description

Care Coordinator - Mental Health Specialist

Positions available in Licking and Knox Counties

Duties: Provide care coordination services to adult clients with mental health and substance abuse issues. Implement monitoring systems, determine client needs, and ensure delivery of needed treatment.

Qualifications: Candidates must possess strong written and oral communication skills and the ability to collaborate with other service providers. A high school diploma with one to three years of care coordination experience is required. An Associate's degree in Human Services or a related field is preferred. A Qualified Mental Health Specialist (QMHS) certification is also preferred. A State of Ohio driver's license and BLS/CPR certification are required.
