Rutgers University is seeking a Nursing Care Coordinator to join our team at the NJMS-Ambulatory Care/Infectious Diseases department. The primary purpose of this role is to coordinate and deliver specialized nursing care services to patients living with and/or at risk for HIV/AIDS.

This position involves collaboration with a multidisciplinary team to optimize patient outcomes and enhance the overall quality of care. Key responsibilities include:

  • Conducting thorough assessments of patients to determine their healthcare needs.
  • Coordinating and monitoring the delivery of medical treatments, medications, and interventions.
  • Providing education and counseling to patients and their families regarding HIV prevention, treatment options, and adherence to care plans.
  • Offering counseling and emotional support to patients and their families, addressing psychological and social aspects of living with HIV.

Requirements for this position include a graduate degree in nursing with a license to practice in the State of New Jersey, and at least two years of clinical nursing experience with familiarity in the use of electronic medical records (EMR). Experience in Infectious Diseases, HIV/AIDS care is preferred.
