Job Summary
ChristianaCare is seeking a skilled Clinical Case Manager to play a critical role in ensuring the delivery of efficient and effective healthcare services. As a Registered Nurse, you will conduct admission and concurrent reviews to evaluate medical necessity and appropriateness of care. You will collaborate with healthcare providers to develop and implement a multidisciplinary plan-of-care that meets patient needs and desired outcomes. Additionally, you will manage observation level of care and work with attending physicians to ensure observation status does not exceed 48 hours. You will also identify system issues that impact care and participate in developing strategies to remove barriers and improve performance. Furthermore, you will monitor efficiencies in scheduling diagnostic procedures and coordinate treatments to achieve clinical, fiscal, and patient satisfaction goals.
Requirements
DE RN licensure or compact state RN licensure. Bachelor's Degree in Nursing. Minimum of 3 years recent experience as a Registered Nurse in acute care, adult care setting. Clinical documentation, performance improvement, case management, or utilization management experience desirable.
