Job Summary:

The Peer Advocate-Swing will serve as a valued member of our interdisciplinary team, working closely with clients to provide support, guidance, and resources to help them achieve their goals.

Key Responsibilities:
  • Build strong relationships with clients, demonstrating empathy and understanding of their unique challenges.
  • Provide consistent support and motivation to clients, encouraging them to engage in behavior modification programs.
  • Maintain accurate and confidential records of client progress and interactions.
  • Initiate regular contact with clients to check in on their well-being and provide support as needed.
  • Accompany clients to referrals for health and social services, ensuring they receive the resources they need.
  • Facilitate referrals to substance use recovery services, healthcare facilities, and other relevant social services.
  • Deliver educational content and skill-building information to clients, helping them develop the skills they need to succeed.
  • Share personal experiences and insights to help clients understand the importance of their own recovery and well-being.
  • Contribute to the ongoing collection of program data and the completion of program-related forms.
  • Assist in the preparation of monthly reports for funding agencies.
Requirements:
  • Proven experience working with individuals experiencing homelessness.
  • High School diploma or GED.
  • Exceptional communication skills, with the ability to engage with clients who may have substance use or other related challenges.
  • Personal experience overcoming challenges similar to those faced by clients.
  • Ability to reliably travel to various community sites and attend appointments on time.
  • A strong desire to make a difference in the lives of homeless individuals.
