Job Summary:

The role of the Patient Navigator involves utilizing clinical expertise, coordination, and advocacy to support patients and their families in navigating the complexities of the healthcare system.

The primary objectives include enhancing patient satisfaction and optimizing care outcomes. The navigator serves as a communication bridge to understand the unique clinical needs, preferences, and concerns of each patient.

Key Responsibilities:

  • Guide patients and families to a wide array of services, amenities, and information to foster healing and ensure a positive patient care experience.
  • Act as an essential member of the multidisciplinary team, providing support to stroke patients and their families from admission through post-discharge follow-up.
  • Utilize expertise in neuroscience care to facilitate safe and smooth transitions for patients from acute care to recovery and beyond.
  • Assist patients with fundamental health education related to stroke care, helping them set and achieve health improvement goals, and manage medications effectively.
  • Collaborate with a multidisciplinary team to enhance comprehensive care and services for patients, while also educating nursing staff and team members on stroke care protocols.

Department Overview:

The Stroke Navigator will report directly to the Stroke Program Manager and will work within the Neurosciences Service Line, which encompasses various specialties including Neurology and Neurosurgery.

Vision:
UCSF Neurosciences aims to maintain its status as a leading center for comprehensive care and prevention of neurological disorders, integrated with research and educational missions.

Required Qualifications:

  • A minimum of 3 years of bedside nursing experience, particularly with neurovascular patients, ICU, or emergency department experience.
  • Acute care nursing experience with advanced clinical knowledge in neuroscience.
  • Strong assessment skills to identify patient needs and make appropriate referrals for specialized support services.
  • Proficient in navigating complex, multi-divisional environments and leading projects from inception to completion.
  • Excellent communication and cultural competency skills to educate patients on sensitive clinical issues.

Preferred Qualifications:

  • Experience in critical care settings.
  • Familiarity with the Joint Commission certification process.
  • Specialty certifications related to stroke care.
  • NIH Stroke Scale Certification.

Licensure/Certification:

  • Bachelor's degree in Nursing or a related healthcare field.
  • Registered Nurse license in the State of California.
  • Current American Heart Association BLS & ACLS certifications.
