Job Summary:
We are seeking a skilled Clinical Care Coordinator to join our team at Sutter Health. As a key member of our healthcare team, you will play a vital role in ensuring seamless navigation for patients, families, and caregivers throughout the continuum of care.

Responsibilities:
Collaborate with physicians and interdisciplinary teams to triage, coordinate, and manage patient care
Educate patients and serve as a point of contact for patients and families
Participate in strategic initiatives to develop, implement, and monitor navigation activities and sustainability

Requirements:
Associate degree in nursing
RN-Registered Nurse of California licensure
5 years recent relevant experience
Knowledge of medical terminology and third-party reimbursement
Excellent verbal and written communication skills
