Job Description
A dynamic and empathetic Care Manager II is needed to join our Apicha Community Health Center team. This role involves delivering high-quality, person-centered care to clients, ensuring their overall well-being and addressing their specific needs.

Key Responsibilities and Qualifications
The ideal candidate will have a Bachelor's degree in a relevant field or equivalent experience. They should be able to work effectively with clients, conduct thorough assessments, and develop personalized care plans. Traveling extensively throughout NYC via public transportation is a requirement. Strong communication skills, both written and verbal, are essential. Fluency in Spanish or an Asian language is preferred.

Salary and Benefits
We offer a competitive salary range of $63,000 - $73,000 per annum, along with comprehensive benefits, including medical, dental, and vision coverage. Opportunities for professional growth and development are also available.
