Position Overview


As a vital member of a collaborative healthcare team at Central Health, the Specialty Care Advanced Practice Clinician (APC) plays a crucial role in delivering direct patient services within their area of expertise, which may encompass fields such as cardiology, neurology, gastroenterology, nephrology, palliative care, or pulmonology.

This role also presents opportunities for the development and oversight of specialized programs aimed at addressing specific health conditions, such as a heart failure initiative.

The APC will work closely with a diverse group of healthcare professionals, including physicians, pharmacists, dietitians, social workers, nurses, and other clinical staff, to meet clinical objectives and enhance quality of care.

In the capacity of Lead APC, this position will oversee and guide other specialty APCs, providing clinical leadership, education, and support.

Collaboration with the clinic's lead physician, nurse manager, and clinic administrator is essential, as is serving as an advocate and liaison for the APC team.
Protected administrative time will be allocated for this role to facilitate care coordination and management tasks.

Key Responsibilities

Essential Duties

  • Under physician supervision, deliver direct patient care within the Specialty scope of Central Health.
  • Develop, promote, and coordinate high standards of clinical practice.
  • Conduct patient examinations, create diagnostic plans, and order necessary tests.
  • Perform routine procedures as clinically indicated within the APC's scope of practice.
  • Prescribe medications, conduct screenings, and coordinate referrals as necessary.
  • Ensure effective communication with patients, healthcare providers, and administrative staff, maintaining cultural and linguistic appropriateness.
  • Acquire a comprehensive understanding of Epic applications utilized at Central Health.
  • Collaborate with primary care providers and other licensed healthcare professionals to ensure patient-centered care aligned with Central Health's policies and goals.
  • Conduct specialty assessments through various examination techniques.
  • Order and execute tests and diagnostic imaging to inform patient care.
  • Diagnose and treat conditions within designated specialty areas, referring patients as needed.
  • Prepare and submit timely and accurate reports and documentation as required.
  • Facilitate collaboration among providers to ensure comprehensive care for eligible patients.
  • Coordinate daily care with team members to optimize quality and efficiency.
  • Perform clinical and administrative tasks with minimal supervision, accountable for achieving organizational goals.
  • Engage with diverse patient populations and their families.
  • Communicate effectively with care coordination teams to prevent readmissions.
  • Demonstrate strong organizational and time management skills in prioritizing responsibilities.
  • Maintain confidentiality and exhibit professionalism in all interactions.
  • Collaborate with the Epic team to enhance expertise in Epic applications.
  • Assist in customizing electronic medical records to improve workflow.
  • Undertake additional duties as assigned.

Qualifications

Minimum Education:
Graduation from an accredited physician assistant program or a master's degree from an accredited nursing program.

Minimum Experience:

A minimum of two years of experience as an advanced practice provider in a specialty setting.


Licensure:
Must hold and maintain an unrestricted license to practice as a Physician Assistant or as a Nurse Practitioner in Texas, along with current DEA certification and CPR certification through an accredited organization.
