Key Responsibilities
  • Engage with patients and their families to discuss end-of-life care and develop care plans for those with chronic illnesses.
  • Independently conduct thorough patient assessments, including history-taking and physical examinations, to formulate appropriate diagnoses and treatment strategies.
  • Facilitate quality of life discussions with patients and their families.
  • Order and interpret necessary diagnostic and therapeutic tests based on patient needs.
  • Collaborate with care coordinators to ensure timely access to ancillary services.
  • Prescribe suitable pharmacological and non-pharmacological treatment options.
  • Work alongside physicians to manage both acute and chronic medical needs of patients.
  • Maintain accurate and efficient patient records and documentation.
  • Educate patients and families on medication management and treatment plans for home care.
  • Demonstrate autonomy in medical decision-making and provide a wide range of diagnostic and therapeutic services.
Qualifications
  • Current RN and NP licensure in the state.
  • Board Certification in a relevant Nursing Specialty/Field.
  • Bachelor's and Master's Degrees in Nursing from accredited institutions.
  • Experience in nursing is essential; prior experience as a Nurse Practitioner is preferred but not mandatory. Critical thinking skills are a must.
  • Knowledge of nursing practices and critical thinking skills applicable to elderly, chronically ill, and recovering patients.
  • Strong leadership, interpersonal, and problem-solving abilities are preferred.
  • Excellent written and verbal communication skills.
  • Demonstrated punctuality, organizational skills, and ability to work collaboratively within a team.
