Key Responsibilities
- Engage with patients and their families to discuss end-of-life care and develop care plans for those with chronic illnesses.
- Independently conduct thorough patient assessments, including history-taking and physical examinations, to formulate appropriate diagnoses and treatment strategies.
- Facilitate quality of life discussions with patients and their families.
- Order and interpret necessary diagnostic and therapeutic tests based on patient needs.
- Collaborate with care coordinators to ensure timely access to ancillary services.
- Prescribe suitable pharmacological and non-pharmacological treatment options.
- Work alongside physicians to manage both acute and chronic medical needs of patients.
- Maintain accurate and efficient patient records and documentation.
- Educate patients and families on medication management and treatment plans for home care.
- Demonstrate autonomy in medical decision-making and provide a wide range of diagnostic and therapeutic services.
- Current RN and NP licensure in the state.
- Board Certification in a relevant Nursing Specialty/Field.
- Bachelor's and Master's Degrees in Nursing from accredited institutions.
- Experience in nursing is essential; prior experience as a Nurse Practitioner is preferred but not mandatory. Critical thinking skills are a must.
- Knowledge of nursing practices and critical thinking skills applicable to elderly, chronically ill, and recovering patients.
- Strong leadership, interpersonal, and problem-solving abilities are preferred.
- Excellent written and verbal communication skills.
- Demonstrated punctuality, organizational skills, and ability to work collaboratively within a team.
