What We're Looking For

We are seeking a detail-oriented and organized Clinical Care Coordinator to join our team. The ideal candidate will have a strong background in nursing and medical practices, as well as laws, regulations, and guidelines that pertain to long-term care. The successful candidate will be responsible for collecting information to complete the MDS, participating in daily PPS meetings, and reviewing the Plan of Care to ensure federal billing requirements are met.

Requirements

The successful candidate will have a Registered Nurse license, a minimum of three years of clinical experience in a healthcare setting, and a minimum of one year of MDS experience.
