Family Service League Inc. is seeking a dedicated and compassionate Case Manager/Family Development Specialist to join our team. As a key member of our Home Base program, you will be responsible for providing direct services to vulnerable populations, including youth and families.
Key Responsibilities- Conduct linkage, coordination, advocacy, and mentoring services to support goal achievement and improve the quality of life for our clients.
- Provide anger management and parenting skills training to youth and their families, promoting healthy relationships and community engagement.
- Focus services on addressing the factors that contributed to the youth's involvement with mental health services, ensuring a comprehensive approach to care.
- Manage a caseload of 14 youth, conducting weekly visits and providing case management services to support their needs.
- Collaborate with skill builders to achieve client goals, referring clients as needed for skill-building services.
- Conduct home visits throughout Suffolk County, utilizing Motivational Interviewing to facilitate positive change.
- Perform other duties as assigned, contributing to the overall success of our Home Base program.
- High school diploma or equivalent required; Bachelor's degree in social work or related field preferred.
- At least 2 years of experience working in human services, with a proven track record of success in working with diverse populations.
- Excellent interpersonal and verbal and written communication skills, with the ability to engage both youth and parents in service delivery.
- Knowledge of community resources, sound judgment, reliability, flexibility, and multitasking skills are essential.
- Proficient computer skills, including Microsoft Office, and a valid and clean NYS Driver's License are required.
