MDS Coordinator Job Summary

The MDS Coordinator is a vital member of our interdisciplinary team, responsible for ensuring the completion of the Minimum Data Set (MDS) assessment and transmission of the assessments to the Center for Medicare and Medicaid Services.

Utilizing the data collected, the MDS Coordinator develops a comprehensive care plan that addresses the emotional, physical, and mental abilities of each patient. This plan is formulated in collaboration with the interdisciplinary team and the family and resident to provide goals for the resident's safety, long-term, and short-term health.

Key Responsibilities:
  • Comply with Facility policies and procedures
  • Comply with nursing department policies and procedures
  • Report witnessed or reported allegations of resident abuse/neglect immediately
  • Treat residents, family members, visitors, and team members with respect and dignity
  • Maintain confidentiality of all resident and family information
  • Report to work on time and as scheduled
  • Conduct initial and periodic comprehensive, accurate, and standardized assessments of each resident
  • Perform administrative duties as necessary
  • Develop, implement, and maintain an ongoing quality assurance program for resident assessment/care plans
  • Ensure comprehensive care plans are completed within twenty-one days of resident admission
  • Participate in Facility surveys (inspections) made by authorized government agencies
  • Lead the Interdisciplinary Care Plan Team in developing comprehensive resident assessments and care plans
  • Participate in discharge plans as necessary
  • Maintain effective, friendly working relationships with health professionals, physicians, consultants, and governmental agencies
  • Conduct or coordinate interviewing of each resident for resident assessment
  • Develop and implement procedures with the Director of Nursing Services to inform assessment team members of newly admitted residents
  • Coordinate development of written plans of care for each resident
Requirements:
  • Valid professional license (RN/LPN) in good standing with the State
  • Valid CPR certification
  • Basic computer skills
  • Ability to read, write, speak, and understand English language
  • Ability to work beyond normal working hours and on weekends and holidays when necessary
  • Ability to assist in evacuation of residents during emergency situations
  • Ability to perform essential position functions with or without reasonable accommodation
