Job Summary

The MDS Coordinator plays a vital role in ensuring the completion of the Minimum Data Set (MDS) assessment and its timely transmission to the Center for Medicare and Medicaid Services. This position requires a high level of attention to detail and organizational skills to develop a comprehensive care plan that addresses the emotional, physical, and mental abilities of each patient.

Key Responsibilities
  • Comply with facility policies and procedures, as well as nursing department guidelines
  • Report allegations of resident abuse or neglect immediately to the Administrator or Director of Nursing
  • Treat residents, family members, and team members with respect and dignity
  • Maintain confidentiality of resident and family information
  • Conduct initial and periodic comprehensive assessments of each resident
  • Develop, implement, and maintain an ongoing quality assurance program for resident assessment and care plans
  • Ensure comprehensive care plans are completed within 21 days of admission
  • Participate in facility surveys and inspections
  • Lead the Interdisciplinary Care Plan Team in developing comprehensive resident assessments and care plans
  • Coordinate discharge plans as necessary
Requirements
  • Valid professional license (RN/LPN) in good standing with the State
  • Valid CPR certification
  • Basic computer skills
  • Ability to read, write, speak, and understand English
  • Ability to work beyond normal working hours and on weekends/holidays as needed
