Responsibilities- Conduct comprehensive medical assessments, diagnose and treat acute and chronic illnesses, and provide ongoing care for patients of all ages.
- Promote health and prevent disease through routine check-ups, screenings, immunizations, and patient education.
- Maintain accurate and timely patient records in compliance with medical and legal standards.
- Manage chronic conditions such as diabetes, hypertension, anxiety, depression, and asthma, developing personalized care plans to ensure effective treatment and monitoring.
- Educate patients and their families on health conditions, treatment options, and preventive measures to promote long-term wellness.
- Collaborate with other healthcare providers, including specialists, nurses, social workers, and behavioral health professionals, to ensure comprehensive and coordinated care.
- Participate in community outreach programs, health fairs, and educational events to promote health awareness and preventive care.
- Participate in quality improvement initiatives within the health center to enhance patient care and outcomes.
- Advocate for patients' health needs, addressing social determinants of health and promoting health equity within the community.
- Participate in on-call coverage as needed (typically one week/month).
- Provide leadership within the clinical team, guiding best practices, participating in quality improvement efforts, and mentoring other healthcare staff.
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution.
- Completion of an accredited residency program in Family Medicine.
- Board certified or board eligible in Family Medicine, or the American Osteopathic Board of Family Physicians.
- Valid medical license to practice in Kansas.
- Valid DEA license.
- Valid Basic Life Support Certification.
- Valid Mental Health First Aid Certification (completed upon hire).
