About the Role

Medi Home Health & Hospice is seeking an experienced and compassionate Full-Time Hospice Aide/CNA to provide support services for our patients in Pulaski, VA.

Job Summary

This role involves providing personal care and activities of daily living assistance under the supervision of skilled professionals. Responsibilities include:

  • Providing personal care and activities of daily living as per the patient's plan of care.
  • Applying safety principles and proper body mechanics to perform techniques of personal and supportive care such as ambulation, transferring patients, assisting with normal range of motion and positioning, and completing household chores.
  • Preparing and providing adequate nutrition and fluid intake, including medically prescribed diets under supervision.
  • Observing and reporting changes in patient condition or expectations of service promptly to a skilled nurse via telephone and documenting communication in clinical notes in the Electronic Medical Record (EMR).

Requirements

To be successful in this role, you will need:

  • A high school diploma or General Education Degree (GED) required.
  • Documented completion of a state-recognized Certified Nursing Assistant course.
  • Valid Certified Nursing Assistant certification as required by the state.
  • Valid driver's license and company-required auto-liability insurance.
