Job Summary

We are seeking an experienced Care Coordination Specialist to join our team. The successful candidate will be responsible for coordinating the transition of care for patients from acute settings to home, ensuring seamless communication between healthcare providers and patients.

Key Responsibilities

  • Coordinate daily Inpatient Notification of admission reports and ensure timely notification of Primary Care Physicians (PCPs) across all IPA's.
  • Retrieve daily Inpatient Discharge Reports and notify PCPs across all IPA's of discharge notifications.
  • Sort discharges, arrange post-discharge follow-up appointments with respective PCPs, and document appointment details in EZCap notes.
  • Fax/Email medical records to PCPs and archive TOC packets when received.
  • Participate in special projects as required and collaborate with the Transition of Care lead in overseeing the Transition of Care Team.

Requirements

  • High school graduate required; Associates degree or higher preferred.
  • Knowledge of Healthcare and Managed Care required.
  • Proficient in MS Office programs (i.e., Word, Excel, Outlook, Access, and PowerPoint).

Salary Range

$65,000 - $85,000 per year, depending on experience.
