Job Overview

The Enrollment RN will be responsible for facilitating enrollment experiences for prospective participants, ensuring a seamless and empowering journey into our PACE program. This role requires strong communication skills, attention to detail, and the ability to work collaboratively with cross-functional teams.

Key Responsibilities
  • Collaborate with Outreach Specialists and IDT teams to coordinate enrollment processes
  • Conduct thorough home visits to assess prospective participants for eligibility and suitability
  • Develop and implement strategies to promote census growth and participant retention
  • Maintain accurate records of all evaluations in CRM software
Requirements
  • Associate's degree from an accredited nursing school; BSN preferred
  • Active Registered Nurse (RN) license in California
  • Current CPR certification or ability to obtain within 30 days
  • 3-5 years of healthcare experience with preference for working with frail elderly populations
  • Valid California driver's license, reliable transportation, good driving record, and auto insurance
