Job Summary:

This role involves providing daily support to adults with intellectual and developmental disabilities. Your responsibilities will include assisting with personal care, meal preparation, transportation, and social activities. You will work closely with each individual to tailor your support and promote their personal growth and independence.

Duties/Responsibilities:

  • Transport and supervise individuals as required to meet employment, recreational, and leisure activities.
  • Ensure vans are inspected regularly and all necessary training is followed for safe transport.
  • Maintain cleanliness of homes, kitchens, bathrooms, windows, bedrooms, and floors.
  • Prepare meals according to individual healthcare plans and manage diets.
  • Provide personal care support, including changing, dressing, and other ADLs.
  • Document and record notes using the Therap system and other programs.
  • Work towards completing goals outlined in individual ISP plans.
  • Conduct evacuation drills and maintain records.
  • Plan and facilitate person-centered outings.
  • Complete behavioral tracking logs as required.
  • Familiarize yourself with agency, state, and federal laws and policies.
  • Participate in required training and development opportunities.
  • Manage medical emergencies according to health service plans.
  • Identify and report maintenance issues to supervisors or through NetFacilities.
  • Adhere to policies governing leave time and punctuality.
