Position Overview:

As an RN Clinician specializing in Long-Term Services and Supports (LTSS), you will play a pivotal role in managing the comprehensive care of our members. This position is designed for individuals residing in designated areas and focuses on optimizing health outcomes across various care settings.

Key Responsibilities:
  • Oversee the management of member cases within your licensure scope, ensuring compliance with state laws and contractual obligations.
  • Provide guidance and oversight to non-RN clinicians involved in member care, fostering a collaborative approach to health management.
  • Develop, monitor, and adjust individualized care plans tailored to meet the unique needs of each member, aiming to enhance their overall health and well-being.
  • Conduct in-person clinical assessments to identify and evaluate the physical, behavioral, and social service needs of members.
  • Recognize members at high risk for complications and coordinate care effectively with the healthcare team.
  • Manage cases involving chronic illnesses and disabilities, ensuring efficient use of health benefits.
  • Gather comprehensive member histories to inform the development of personalized care plans.
  • Set collaborative short- and long-term goals with members, families, and healthcare providers, identifying opportunities for alternative care levels or waiver programs.
  • Take responsibility for the creation and implementation of care plans, ensuring members have access to necessary services.
  • Facilitate authorizations and referrals for services as needed, adhering to benefit structures.
  • Collaborate with Medical Directors and interdisciplinary teams to formulate effective treatment plans.
  • Support problem-solving efforts related to provider, claims, or service challenges.
  • Supervise and direct the activities of LPNs, LSWs, and other licensed professionals, ensuring quality service coordination.

Qualifications:
  • Registered Nurse (RN) with a minimum of three years of experience in case management or service coordination for individuals with chronic conditions.
  • Advanced degrees in Health or Nursing (MA/MS) are preferred.
  • Ability to travel to various work sites as required.
