The Acute Care Program at AltaPointe Health is dedicated to providing comprehensive support for children and adolescents aged 5 to 18 who are facing significant emotional or behavioral challenges. These individuals often require more intensive treatment after previous interventions have not yielded positive outcomes. Our facility ensures a nurturing and structured environment, offering therapeutic services tailored to each child's developmental needs, available around the clock.
This program also extends support to families and caregivers, fostering and enhancing connections with their children throughout the treatment and discharge phases. We cater to children and adolescents with co-occurring mental health and substance use disorders through a specialized service track. Involvement from families, caregivers, educational personnel, and other relevant parties is integral to the treatment and discharge planning process.
Key ResponsibilitiesAs an LPN in our healthcare setting, you will play a crucial role in the care and management of our consumers. Your responsibilities will include:
- Facilitating the admission process for consumers, which encompasses completing necessary health screenings, medication documentation, and vital sign assessments.
- Monitoring consumer behavior during group activities, documenting observations, and collaborating with the treatment team to ensure continuity of care.
- Delivering nursing care and guiding behavioral staff as required.
- Assisting consumers with personal hygiene and daily living activities.
- Administering medications while adhering to established protocols and providing education regarding their use and potential side effects.
- Maintaining effective communication with physicians about the physical and mental health status of consumers.
- Participating actively in the interdisciplinary treatment planning process.
- Responding to emergencies as needed, following established policies and procedures.
- Documenting care and observations accurately and in a timely manner, ensuring compliance with facility policies.
- Engaging in professional development and maintaining licensure requirements.
This role involves various physical activities, including:
- Frequent standing, walking, and sitting, with the ability to adapt to the needs of consumers.
- Occasional lifting of up to 50 pounds to assist with daily living activities.
- Ability to respond quickly in emergency situations, including physical interventions when necessary.
Current licensure as an LPN in Alabama is required.
