Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team. As a LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home.

Responsibilities
  • Deliver patient care services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  • Instruct patients and their families on proper care and maintenance of a healthy environment.
  • Monitor patient progress, record observations, and report to the supervising nurse and/or physician.
Requirements
  • Current LPN licensure in the state of practice.
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
  • CPR certification.
