Job Summary

We are seeking a skilled Licensed Practical Nurse LPN to join our team at Ambercare. As a key member of our healthcare team, you will provide high-quality patient care and support in the comfort of clients' homes.

Key Responsibilities
  • Perform assessments and simple medical nursing procedures under the guidance of an RN Supervisor.
  • Implement nursing care based on plans developed by a Registered Nurse (RN) in accordance with physician's orders.
  • Provide patient and family teaching according to plans outlined by the RN.
  • Observe, assess, and record patients' physical and mental conditions, reporting any changes.
  • Administer medications as instructed and as ordered by the physician and instructed by the RN.
  • Document patients' files accordingly.
  • Work effectively with patients, families, and healthcare team members to coordinate patient care.
  • Maintain confidentiality and adhere to all regulations, policies, procedures, and standards.
Requirements
  • Licensed as a vocational/practical nurse in the state of practice.
  • At least two (2) years of clinical practical nursing experience.
  • Trach and Vent experience preferred.
  • Ability to make home visits and provide nursing care in the home environment.
  • Ability to complete necessary documentation appropriately, legibly, and timely.
  • Reliable transportation, current driver's license, and appropriate automobile insurance, or ability to take public transportation.
