Job Summary: We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Ambercare. As a Home Health Nurse, you will be responsible for providing high-quality nursing care to clients in their homes, under the guidance of our RN Supervisor. Your primary focus will be on implementing nursing care plans developed by our Registered Nurses (RNs) in accordance with physician's orders.

Key Responsibilities:
  • Perform selected nursing procedures, including tracheostomy care, colostomy care, enemas, douches, catheterizations, dressing changes, and phlebotomies.
  • Provide patient and family teaching according to the plan outlined by the RN.
  • Observe, assess, and record patient's physical and mental condition and report any changes.
  • Administer medications as instructed and as ordered by the physician and instructed by the RN. Document the patient's file accordingly.
  • Work with patient, family, and healthcare team members effectively to coordinate patient care.
  • Maintain a high degree of confidentiality at all times due to access to sensitive information.
  • Follow all Medicare, Medicaid, and HIPAA regulations and requirements.
  • Abide by all regulations, policies, procedures, and standards.

Requirements & Competencies:
  • Licensed as a vocational/practical nurse in the state of practice.
  • At least two (2) years clinical practical nursing experience.
  • Trach and Vent experience preferred.
  • Ability to make home visits and provide nursing care in the home environment.
  • Ability to complete necessary documentation appropriately, legibly, and timely.
  • Must have reliable transportation, current driver's license, and appropriate automobile insurance; or ability to take public transportation.
