The ideal candidate will have a strong background in physical therapy, excellent communication skills, and a passion for delivering patient-centered care.
Key Responsibilities- Assess and evaluate patients' physical therapy needs and develop individualized plans of care.
- Provide physical therapy services to patients in their homes, including exercises, stretches, and other interventions to improve mobility and function.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordinate services.
- Communicate effectively with patients, families, and caregivers to educate them on physical therapy techniques and promote self-care.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in quality improvement initiatives and contribute to the development of best practices in physical therapy.
- Master's degree in Physical Therapy from an accredited program.
- Current licensure to practice as a Physical Therapist in the state of California.
- At least 2 years of experience as a Physical Therapist, preferably in home health or community-based settings.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
