Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at The Care Team Home Health & Hospice. As a LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver exceptional care and support.

Key Responsibilities
  • Provide skilled nursing care to patients in their homes, following their individualized care plans and adhering to our agency's policies and procedures.
  • Conduct thorough assessments of patients' physical and emotional needs, documenting findings and communicating with the care team to ensure seamless care coordination.
  • Administer medications and treatments as ordered, maintaining accurate records and adhering to medication management protocols.
  • Collaborate with patients, families, and caregivers to educate them on self-care techniques, disease management, and wellness strategies.
  • Participate in interdisciplinary team meetings to discuss patient progress, address concerns, and develop care plans.
  • Ensure compliance with regulatory requirements, including HIPAA, infection control, and safety protocols.
Requirements
  • Current licensure as a Licensed Practical Nurse (LPN) in the State of Michigan.
  • Minimum one year of experience in a home health care setting, preferably in a hospice or palliative care environment.
  • Knowledge of general nursing practice, including medication management, wound care, and patient education.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to maintain accurate records, adhere to agency policies, and meet regulatory requirements.
