Job Summary

Dignity Health - Northridge Hospital Medical Center seeks a skilled Clinical Social Worker to provide clinical social work services to adult and geriatric patients and their families in the inpatient and outpatient Behavioral Health departments. The ideal candidate will have a Master's degree in social work from an accredited program and be licensed as a Clinical Social Worker (LCSW) in the state of California.

Responsibilities
  • Provide clinical social work services to patients and their families, including psychotherapeutic techniques and psychosocial treatment plans.
  • Develop and implement age and disability appropriate psychosocial treatment plans related to personal and family adjustment and continuity of care needs.
  • Provide information, referrals, and liaison activities with third-party payors and community resources necessary for patients' post-hospital needs.
  • Participate in conferences and staffings to provide information and make recommendations for patients' treatment, discharge planning, and milieu management.
  • Prepare timely medical record documentation of social work services.
Qualifications
  • Masters degree in social work from an accredited program.
  • Advanced degree with field placement or job experience in hospital health mental health or related setting.
  • Must demonstrate knowledge of the principles of growth and development over the life span.
  • Must possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs.
  • Must be California State Licensed Clinical Social Worker (LCSW) required.
