Outpatient Case Manager

The Outpatient Case Manager is a critical member of our healthcare team, responsible for coordinating and managing the care of high-risk patients in an outpatient setting. This role requires a strong clinical background, excellent communication skills, and the ability to work effectively with patients, families, and healthcare providers.
The ideal candidate will have a strong understanding of the healthcare system, including insurance coverage, medical billing, and regulatory requirements. They will also possess excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
As a key member of our team, the Outpatient Case Manager will be responsible for:
- Conducting comprehensive assessments of patients' medical and psychosocial needs
- Developing and implementing individualized care plans that address patients' unique needs and goals
- Coordinating with healthcare providers, including physicians, nurses, and other specialists, to ensure seamless care transitions
- Providing education and support to patients and families, promoting self-management and empowerment
- Identifying and addressing potential barriers to care, including social determinants and access to resources
The Outpatient Case Manager will work closely with our interdisciplinary team to ensure that patients receive comprehensive, patient-centered care. This role requires a strong commitment to delivering high-quality care, with a focus on patient safety, satisfaction, and outcomes.
Requirements
- Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program
- Current CA RN or LVN license, current CPR certification, and valid CA Driver's license
- 3 years acute care or case management experience preferred
- 2-3 years of utilization or HMO experience preferred
- Typing 40 words per minute with accuracy
- Knowledge of computers, faxes, printers, and all other office equipment
- Knowledgeable in MS Office Programs (i.e., Word, Excel, Outlook, Access, and PowerPoint)
