Harm Reduction Program Manager

Fair Haven Community Health Care is a dynamic and forward-thinking community health center that provides high-quality care to multiple generations. We are seeking a Harm Reduction Lead to join our team and contribute to our mission of improving the health and social well-being of the communities we serve.
The Harm Reduction Lead will serve as the on-site administrative lead at our Harm Reduction Center and will work closely with clinical and operations leadership to align our approach and strategy with our mission and strategic plan. Key responsibilities include:
- Serving as a community-facing lead for the Harm Reduction Center
- Ensuring that process- and outcomes-measures pertinent to the Harm Reduction Center are collected, collated and analyzed to inform refinements and continuous quality improvement
- Reporting on the progress of the Harm Reduction Center to internal and community stakeholders
- Collaborating with FHCHC clinical and operations leadership on sustainability planning for harm reduction-related activities
- Providing individual and/or group recovery-oriented activities
- Developing person-centered recovery plans with stage-wise treatment goals and objectives that are measurable; observing and recording clients' progress towards goals
- Providing individual and group sessions in community settings within the psychiatric rehabilitation skill-building model
- Consulting with behavioral health and medical clinicians to assure quality of care
Qualifications include a Bachelor Degree and Peer Recovery Support Certification in the state of Connecticut. The Harm Reduction Lead will supervise the Care Coordinator for Harm Reduction and create a culture of humility, cultural responsiveness, and hospitality at the Center.
