Job Summary
We are seeking a Support Coordinator to join our team. The successful candidate will provide direct support to individuals with disabilities, promoting their independence and well-being. This is an exciting opportunity to make a real difference in the lives of others.
About the Role
- Provide direct support to individuals with disabilities, including assistance with daily living tasks and personal care
- Develop and implement individualized support plans to promote independence and well-being
- Work collaboratively with colleagues and community partners to ensure seamless support services
- Participate in ongoing training and professional development to enhance skills and knowledge
Requirements
- BA or 1-2 years experience in a related field (e.g. human services, social work)
- Experience working with individuals with disabilities, particularly those with emotional and behavioral needs
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
