Job DescriptionAt All Med Search, we are seeking a highly skilled and experienced Registered Nurse to fill the role of Director of Case Management. This leadership position plays a critical role in ensuring the coordination and delivery of comprehensive patient care services. The ideal candidate will possess strong leadership and communication skills, with a proven track record of managing case management teams and driving the development of case management programs that enhance patient outcomes while optimizing resource utilization.Key Responsibilities:
  • Provide strategic leadership to the case management team, ensuring a collaborative and cohesive work environment.
  • Develop and implement case management programs to optimize patient care and resource allocation.
  • Collaborate with other healthcare professionals to establish and maintain standards and best practices.
  • Monitor and evaluate care plans for their effectiveness and adjust as needed.
  • Ensure compliance with all applicable laws and regulations, including CMS guidelines.
Qualifications:
  • Current and valid Registered Nurse (RN) license.
  • Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred.
  • Certification in Case Management (CCM or ACM) strongly preferred.
  • A minimum of 5 years of experience in healthcare, with at least 2 years in a case management leadership role.
  • Proficiency in healthcare informatics and electronic health records (EHR) systems.
