About the Role

We are seeking a skilled RN Care Coordinator to join our team at Dignity Health - California Hospital Medical Center. As a key member of our care coordination team, you will play a vital role in ensuring the seamless progression of care for our patients.

Responsibilities
  • Oversee the progression of care and discharge planning for identified patients requiring these services.
  • Perform care coordination to meet individual health needs while promoting quality of care and cost-effective outcomes.
  • Collaborate with utilization management, nursing, physicians, ancillary departments, insurers, and post-acute service providers to progress care toward optimal outcomes.
  • Advocate for patients and families by identifying, valuing, and addressing patient choice, spiritual needs, cultural, language, and socioeconomic barriers to care transitions.
Qualifications
  • Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of 1 year experience.
  • California RN license
  • AHA BLS card
  • LA City Fire Card required within 90 days of hire.
  • Ability to pass annual Inter-rater reliability test for Utilization Review product(s) used.
  • Able to apply clinical guidelines to ensure progression of care.
  • Must have critical thinking and problem-solving skills.
  • Collaborate effectively with multiple stakeholders
  • Professional communication skills.
  • Understand how utilization management and case management programs integrate.
  • Ability to work as a team player and assist other members of the team where needed.
  • Thrive in a fast-paced, self-directed environment.
  • Knowledge of CMS standards and requirements.
  • Proficient in prioritizing work and delegating where indicated.
  • Highly organized with excellent time management skills.
  • Excellent customer service and presentation skills are a must.
  • Strong interpersonal and written communication skills are essential.
Preferred Qualifications
  • Graduate of an accredited school of nursing (Bachelor's Degree in Nursing (BSN)) or related healthcare field.
  • At least five (5) years of nursing experience.
  • Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification preferred
  • Knowledge of managed care and payer environment preferred.
Pay Range

$73.44/hour
